Job Summary: The Community Bases Case Management (CBCM) RN shall be a member the multi-professional team responsible for conducting psychiatric and medical assessments, basic health care, education, and coordination of medical needs; coordination of appointments/referrals with community physicians, manage, and monitor psychotropic medication administration under the supervision/order of the psychiatrist. The RN shall have an active role in the provision of case management and rehabilitation Services. The RN shall work with the consumer and their support system on the management of symptoms, the development of wellness responses to co-morbid conditions, and development of CM team skills to manage complex health conditions, under the direct supervision on the Care Hawaii, CBCM Team Leader and/or CBCM Psychiatrist.
Minimum Education: Must be a Licensed Registered Nurse in the State of Hawaii, Masters, Bachelors or Associate degree, and have two (2) years experience, in mental health nursing.
Minimum Field-of-Expertise: Strong clinical skills and experience demonstrating competence in problem-solving, critical thinking, collaboration, and clinical competencies in providing education and training to staff members.
Required Experience: Must have a minimum of two (2) years of experience in mental health; interest and knowledge of current trends and new developments in the mental health field for the adult target population. Inpatient/outpatient psychiatric nursing preferred.
